Houston Methodist Sponsorship Request Form
Houston Methodist prides itself on being a leader dedicated to the health and well being of the Houston community. Thank you for your interest in engaging Houston Methodist on your organization's community inititiave. Please follow the instructions below if you are a first time applicant.
Step 1) If this is your first time requesting sponsorship support, please click the Sign Up button on the right hand side of this page to create an account.
Step 2) Once you complete the sign up process, you will receive an email to confirm your registration. Make sure to check your spam box just in case. Follow the instructions in the confirmation email to get started.
Step 3) Once you have gained access to your account, please answer the series of questions so that we may assess your request. You will need to upload the required formal request letter to complete and submit your application.
